Our dedicated After-Hours Team provides care for hospice patients in the community during the evening and weekend hours.  They work from home, visit patients (as needed), and are an essential piece of Casa's mission to provide superior care (day or night).

Position Summary:


The After-Hours RN plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The After-Hours RN responds to patient/family/caregiver needs during the evening, night or weekend hours of operation.




Patient Care

  1. Completes an initial assessment of patient and family when assigned to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
  2. Provides professional nursing care by utilizing all elements of nursing process.
  3. Assesses and evaluates patient’s status by:
    • Re-evaluating patient/family/caregiver needs
    • Revising the Plan of Care as necessary
  4. Initiates the Plan of Care on patients admitted during on-call hours, and makes necessary revisions as patient status and needs change.
  5. Contributes to the interdisciplinary plan of care which establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  6. Initiates appropriate nursing procedures in response to patient’s needs and changes in condition. Administers medications and treatments as prescribed by the physician in the patient’s Plan of Care.
  7. Counsels the patient and family in meeting nursing and related needs.
  8. Provides health care instructions to the patient as appropriate per assessment and plan.
  9. Responds to patient/family needs during the hours of on-call and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.


  1. Completes, maintains, and submits accurate and relevant clinical notes regarding the patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate in electronic record, at point of service.
  2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physician’s orders as required, enters information in the electronic record.


  1. Graduate of an accredited school of nursing, currently licensed in Arizona.
  2. Excellent observation skills, verbal and written communication skills, and problem-solving skills.
  3. Excellent computer skills; experience with electronic health records.
  4. Current CPR certification.
  5. Valid Arizona drivers license.
