Position Summary:
Our Admission RN's play a crucial role and are an essential part of the team of individuals who are committed to guiding patients and their loved ones through the end of life journey with knowledge, compassion, and the highest ethical standards. The Admission RN is often the first hospice personnel to visit a patient. They spend a lot of time with patients and their caregivers explaining this hospice philosophy and developing a plan of care. They are responsible for assessing a patient's need and readiness for hospice and consults with the hospice physician to accept the patient into hospice care.
Position requires a mid-day work schedule and alternating weekend availability.
The Admission RN is responsible for all aspects of the admission process for new patients. They are a member of the interdisciplinary team, reporting to the Admissions Supervisor for assigned functions related to admissions of patients, provision of services information and liaison with inpatient and outpatient institutions and groups identified as sources of referrals consistent with Casa de la Luz policies and procedures. They are responsible for the assessment, planning and coordination of services for admission of patients to Casa de la Luz's program of care.
