Position Summary:


The licensed medical social worker is responsible for the coordination of care for individuals receiving palliative care services. He/she follows standards of care for licensed medical social work services; provides medical social work services to home bound or facility patients and their family members. Responsibilities will include psychosocial assessment, counseling, education and community resource assessments and coordination; referrals, assisting with final arrangements, as well as long-term care planning.



  1. Coordinates care and services in collaboration with the Palliative Care team and other healthcare professionals.
  2. Assesses the individual psychosocial status and needs of patients/families/caregivers related to the patient's serious medical condition and environment. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
  3. Carries out social evaluations and plans intervention based on evaluation findings. Counsels patient/family/caregivers as needed in relationship to stress, and other identified coping difficulties.
  4. Counsels the patient and family to assist with determination of patient/family goals and wishes given the limitations of the serious illness and care-giving demands.
  5. Assists with clarifying, understanding and completion of advance directives.
  6. Completes, maintains and submits accurate and relevant electronic clinical notes at point of service, on all patients.
  7. Provides information and referral services for organization patients/families/caregivers regarding practical and environmental needs.
  8. Provides information to patients/families/caregivers and community agencies.
  9. Serves as liaison between patients/families/caregivers and community agencies.
  10. Maintains collaborative relationships with organization personnel to support patient care.
  11. Maintains and develops relationships with public and private agencies as resources for patient and personnel
  12. Participates in the development of the Palliative Plan of Care and attends team meetings and case conferences as assigned.
  13. Assists physician and other team members in understanding significant social and emotion­al factors related to health problems and issues related to limited life expectancy.
  14. Participates in discharge planning when needed. Assists patient/family/caregiver with securing durable power of attorney and with funeral arrangements, as needed.
  15. Performs community education presentations.
  16. Supports hospice social workers as needed.
  17. Participates in monthly on-call shift rotation.
  18. Position requires additional report to Palliative Care team leader.


  1. A graduate of a Master's program in social work accredited by the Council on Social Work Education.
  2. Current licensure as a Licensed Clinical Social Worker (LCSW) in the State of Arizona.
  3. Advanced Palliative and Hospice Social Worker Certified (APHSW-C) preferred.
  4. Minimum of one year's experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
  5. Experience in a home health care preferred.
  6. As required by the State of Arizona, a licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

Skills and Abilities Required

  1. Demonstrates excellent verbal and written communication skills.
  2. Demonstrates good organizational skills.
  3. Ability to prioritize and manage time effectively.
  4. Excellent computer skills and working knowledge of Microsoft Office software applications (Work, Excel, PowerPoint, Outlook, etc.).
