$14.00/hr-$14.50/hr, DOE

Position Summary:


The Intake Coordinator is responsible for scheduling assessment visits for all inquiry and referral callers, and following-up on all pending patients when due. This person would: initiate private insurance authorization procedures, facilitate equipment and other admission orders as needed, enter prospective and admitted patient demographic information and status updates in the patient database, facilitate processing of registration and admission documents and admissions from initial referral call through transfer to appropriate care team and other duties as requested, and maintain positive relationship with community referral sources.



  1. Coordinates/dispatches admission RNs for assessment visit within required time, following referral call.
  2. Obtains all required information and accurately completes all documentation, forms and logs for referral calls and admissions processing.
  3. Communicate all prospective patient updates to appropriate referral sources.
  4. Maintains positive relationships with community referral.
  5. Communicates with referral source to process all certifications, orders, and other required admission information.
  6. Orders new patients supplies and Durable Medical Equipment.
  7. Schedules transport for new patients.
  8. Proficiently uses EMR to log all communications, orders and transports for referred and new patients.
  9. Completes initial verification of insurance eligibility and benefits and communicates with accounting office.
  10. Makes follow-up calls for referral inquiries within required time.
  11. Attends periodic training on admissions processing.
  12. Meets minimum performance standards for conversion of calls to initial visits.
  13. Prepares status reports for on-call staff.



  1. High school diploma or equivalent
  2. Excellent verbal and written communication skills
  3. Excellent customer service skill and phone etiquette.
  4. Ability to work independently while functioning as a member of a team.
  5. Proficient keyboarding/computer operation skill.
  6. Understanding of issues of confidentiality and privacy
