    Job Description Summary:

     

    A compassionate individual with a passion for helping the terminally ill, the Nurse Practitioner works under the direction of the hospice Medical Director and is responsible for evaluating patient eligibility for hospice care; utilizes knowledge of palliative care to manage individual patient symptoms associated with terminal illness; provides assessments in a variety of patient residential settings including homes, skilled nursing facilities, and Casa’s inpatient unit; acts as a resource to clinical teams and works in collaboration with the IDT to evaluate, implement and change the plan of care as necessary to meet patient/family needs and goals consistent with Casa de la Luz Hospice policies and state and federal regulations.

    Responsibilities

    1. Works collaboratively with the care team to elicit and address patient and family goals.
    2. Performs holistic assessments to guide patient/family discussions of care plan and counsels patients/families regarding care options.
    3. Addresses symptom management needs in a variety of settings including private homes, skilled nursing facilities, Adult Care Homes, Assisted Living Homes, Kanmar Residential Home and Casa inpatient unit, working under the supervision of the hospice medical director.
    4. Evaluates patient eligibility for certification of terminal status and documents according to Medicare standards.
    5. Effectively utilizes multiple modes of communication, including the electronic record, to assure documentation is complete and team members are advised of comprehensive medical history, assessment findings, goals of care, and care plan. Documents comprehensive medical history, identified pertinent patient information and progress, medications and diagnostic studies as required, and according to required timelines.
    6. Follows established hospice policies and procedures as well as Standards of Care and regulatory requirements.
    7. Acts as a resource to clinical staff; orients, instructs and trains personnel.
    8. Shares on-call rotation with hospice physicians.
    9. Attends physician meetings and other meetings as directed by Medical Director or Chief Clinical Officer.
    10. Consults Medical Director when plan of care is outside of standardized practice and protocols, and specialty consults are needed to manage patient’s care.
    11. Presents or arranges for orientation and educational presentations for interdisciplinary team members involved in patient care.
    12. Establishes, builds, and develops effective working relationships with staff and community referral sources. Participates in public relation and community activities that promote organization’s role as an effective member of the hospice and health care delivery system.
    13. Participates in community programs for the purpose of providing education and information to members of the medical community.
    14. Complies with all policies, procedures and requests; adheres to Casa’s Mission, Core Values and Job Non-negotiables.
    15. Other assignments as assigned by Chief Clinical Officer.

    Qualifications

    1. Current Arizona registered nurse licensure in good standing with Advance Practice certification. Must have prescribing and dispensing authority.
    2. Graduate of nurse practitioner program from an accredited university (MSN or Doctorate in Nursing).
    3. Previous nursing experience in hospice or palliative care setting preferred.
    4. Prefer advanced certification in hospice and palliative care nursing.
