Position Summary:

The Team Assistant provides a variety of administrative support functions that contribute to the overall efficient operation of a designated clinical team(s). Primary responsibilities include initiating and responding to phone/email communications, data entry, tracking data, processing vendor requests and providing general administrative support.

Responsibilities

The responsibilities of the Operations Team Assistant include:

  1. Records and enters new admissions, level of care, updates and discharges into computer in a timely manner.
  2. Assists in the appropriate assignment of staff based on client needs, staff qualifications and availability.
  3. Maintains tracking mechanism to assure timely data updates to patient plans of care.
  4. Coordinates with vendors on requests for labs, x-ray service, outpatient hospital procedures, etc.
  5. Assists Patient Services Supervisors (PSS) with monitoring of timely data entry, provides exception reports for PSS follow-up. Sends information to physicians, as delegated by PSS.
  6. Assists PSSs with preparing for IDT meetings, including scribing/printing reports and medication lists and notifying team of recertification dates.
  7. Assures Medicare Part D forms are completed accurately and in a timely manner.
  8. In conjunction with PSSs, assists with all new staff orientation schedule. Serves as non-clinical contact during orientation process.
  9. Reviews timesheets to ensure readiness for PSS approval, including notifying team members of additional action needed.
  10. Manages phone/email communications effectively and efficiently. Answers incoming telephone calls. Takes messages and/or directs calls to appropriate person. Communicates information via text, email, phone or fax in an accurate and timely manner. Uses discretion in handling difficult calls and refers to PSS when appropriate.
  11. Assists MSWs with discharge and/or respite paperwork, including mailing and faxing documents necessary for discharge and/or respite care.

Qualifications

  1. High school diploma or equivalent.
  2. Minimum 1 year medical office experience or previous hospice experience required. 
  3. Excellent communication and customer relation skills to interact with others in a helpful, cooperative and effective manner.
  4. Ability to give and follow written and oral instruction.
  5. Skilled in organizing and prioritizing work and managing multiple tasks and assignments.
  6. Ability to manage time well, meet deadlines and perform assigned duties with attention to detail, speed, accuracy and follow-through with minimal supervision.
  7. Ability to work effectively in a fast paced environment with frequent interruptions.
  8. Ability to gather and interpret data from different sources and problem solve.
