Position Summary:


The RN Case Manager Float plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. The Case Manager Float will be assigned to patients living in their homes and assisted living facilities. The Case Manager Float will be required to drive their personal vehicle to patient visits (mileage is reimburseable according to company policy).






The RN Case Manager Float is responsible for assisting with planning, organizing and directing hospice care. The Case Manager Float will administer medications and treatments as prescribed by the physician in the patient's Plan of Care, counsel the patient and family in meeting nursing and related needs, provide health care instructions to the patient as appropriate per assessment and Plan, and assist the patient with the activities of daily living, facilitating the patient's efforts toward self-sufficiency and comfort care. The Case Manager Float will work in cooperation with the family/caregiver and hospice interdisciplinary team members to meet the needs of the patient/family/caregiver. 


  1. Graduate of an accredited school of nursing.
  2. Current Arizona RN license and CPR certification.
  3. Current/valid drivers license and automobile insurance that meets the requirements of the State of Arizona.
  4. Recent nursing experience (acute care, home health and/or hospice is PLUS!).
  5. Previous experience using electronic medical records.
  6. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  7. Proficient computer skills.
