The RN Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. The Case Manager will be assigned to patients living in their homes and assisted living facilities. The Case Manager will be required to drive their personal vehicle to patient visits (mileage is reimburseable according to company policy).
The RN Case manager is responsible for planning, organizing and directing hospice care. The Case Manager will complete an initial assessment of patient and family to determine hospice needs, provide a complete physical assessment and history of current and previous illnesses, write and initiate Plan of Care, and re-evaluate needs of patient/family/caregiver regularly and revise the Plan of Care as necessary. The Case Manager will administer medications and treatments as prescribed by the physician in the patient's Plan of Care, counsel the patient and family in meeting nursing and related needs, provide health care instructions to the patient as appropriate per assessment and Plan, and assist the patient with the activities of daily living, facilitating the patient's efforts toward self-sufficiency and comfort care. The Case Manager will work in cooperation with the family/caregiver and hospice interdisciplinary team members to meet the needs of the patient/family/caregiver.
